Even sadder cliffa is that, I've been mulling over your proposed idea for the "under" approach ever since you made it and, I think I've an idea that might make it work.Rob, I've been busy mulling this over (sad isn't it) and have come up with another idea which would be aesthetically pleasing on the eye and I think would work. I suppose it all come down to space below the knuckle when it's fully at the end of the stroke. You mentioned you need 40mm movement at the toe piece, but remember that can be reduced by changing the lengths of the two linkage arms.
I think both the gear lever and linkage can work "under".

I've never really liked that "loop over the top" look anyway. Functional though...
